As a primary care physician and USCIS-designated civil surgeon, Dr. Jorge E. Monroy Avella is uniquely qualified to provide immigration medical examinations required for your application for permanent residency with the USCIS, commonly referred to as a “Green Card.”

As a primary care physician and USCIS-designated civil surgeon, Dr. Jorge E. Monroy Avella is uniquely qualified to provide immigration medical examinations required for your application for permanent residency with the USCIS, commonly referred to as a “Green Card.”
A physical examination, reviewing vaccination records, lab findings, medical history, and filling out form I-693 are all included.
Immigration Physicals are completed in two phases. During your first visit, Dr. Monroy Avella will go over your medical history, immunization records, physical examination, and request any required lab work. We will arrange a final appointment when the results of any testing are received so that you may pick up a sealed official report that needs to be submitted, together with a copy of your documentation.
What should I bring to my appointment?
Please bring identification or a passport issued by the government, a document with your alien registration number or USCIS number, and any applicable vaccination records to your appointment.
We might be able to use vaccination records that you have from another nation.

What if one of my vaccines is missing?
Please include any copies of your overseas or domestic vaccination records. There is no issue if you don’t have it. All International immunization records must first be certified by the FLORIDA DEPARTMENT OF HEALTH prior to your visit with us.
We recommend contacting the Health Department for any missing shots or a nearby drug stores for all other vaccines. To complete your paperwork for vaccines in series, you need to have at least the first vaccination in the series.
Under the immigration laws of the United States, a noncitizen who applies for an immigrant visa abroad, or who seeks to adjust status to that of a lawful permanent resident while in the United States, is required to receive vaccinations to prevent the following diseases:
● Mumps
● Measles
● Rubella
● Polio
● Tetanus and Diphtheria Toxoids
● Pertussis
● Haemophilus influenzae type B
● Hepatitis B
● Varicella
● Flu; and
● Any other vaccine-preventable diseases recommended by the Advisory
Committee for Immunization Practices (ACIP).
The ACIP is an advisory committee to the Department of Health and Human Services (HHS)/Centers for Disease Control and Prevention (CDC) that recommends immunizations for the general U.S. population. Since Dec.14, 2009, when the ACIP recommends new vaccines for the general U.S. population, CDC assesses whether these vaccines should be required for immigration purposes on a regular and on an as-needed basis according to specific criteria set by CDC.
CDC is responsible for publishing the Technical Instructions for Civil Surgeons. These documents and the HHS regulations set the requirements for the immigration medical examination and are binding on civil surgeons (See the CDC Vaccination Technical Instructions for Civil Surgeons and 42 C.F.R. Section 34.3). The Technical Instructions include a vaccination component, specifying how the civil surgeon has to conduct the vaccination assessment. The civil surgeon records the results of the immigration medical examination, including the results of the vaccination assessment, on USCIS Form I-693.
Who pays for the vaccinations?
You are responsible for paying the appropriate fee for all vaccinations.
What tests are required as part of Immigration exam?
1) Tuberculosis- for ages 2 and older- Blood test- Quantiferon Gold. If positive a Chest X-Ray will be required.
If the chest X-ray shows active TB lesion(s) or suspicious finding(s), the applicant is mandatorily referred to the county health department for further evaluation/treatment.
If the applicant is pregnant, Chest x-ray is still required or you could delay the x-ray and complete the I-693 until after delivery.
2) Syphilis- for ages 18-44 yrs old high risk individuals. – Blood Test- RPR
3) Gonorrhea urine test for 18-24 yrs old or high risk individuals.
Blood Test
A serologic blood test for HIV (AIDS) and Syphilis (RPR) is necessary for
applicants who are 15 years of age or older. The civil surgeon may, at his or her discretion, request HIV or syphilis testing from applicants under the age of fifteen, or if there is cause for concern over the likelihood of infection. For every applicant who takes the HIV test, post-test counseling will also be given if it is determined that you are HIV positive.
Insurance may cover your blood analysis, you are responsible for any outstanding balances.
If you don’t have insurance, we can refer you to labs with discounted rates.
The civil surgeon must give you the completed Form I-693 in a sealed envelope for you to submit to USCIS. Do not accept the form from the civil surgeon unless it is in a sealed envelope. USCIS will return your Form I-693 to you if it is not in a sealed envelope or if the envelope is opened or altered in any way. The civil surgeon should also give you a copy of the completed Form I-693 for your records.
